Upon arrival, Ernest was greeted by the Concierge, Rehab Team, and Nursing team, who ensured he had everything he needed to start his voyage to renewal here at Cortlandt Healthcare. The Concierge assisted by making sure Ernest received everything during his stay, which included proper food/dietary options, making sure he received and was brought to recreational activities if he wished to attend, and making sure he was receiving medications on time as well as assistance when needed from the nursing team. Ernest was assessed by our Rehabilitation team and received short and long-term goals to monitor his progress during his rehab process. Physical Therapy’s short-term and long-term goals included ambulating on level surfaces, car transfers, and bed mobility tasks. Occupational Therapy’s short-term and long-term goals include functional activities of choice, toileting tasks, and practicing lower body dressings.

During Ernest’s stay, he received both Physical and Occupational Therapy. Starting with Physical Therapy, Ernest progressed to maximum assist with bed mobility tasks to minimum assist. He could ambulate on level surfaces starting at 0 feet and ending at 40 feet with minimum assist. Ernest performed car transfer tasks with supervision to minimum assist. As far as Occupational Therapy goes, Ernest went from 0 to 5 minutes of functional activities of choice. Ernest went from maximum to minimum assist with toileting tasks. Ernest went from maximum assist to moderate assist with lower body dressings while using adaptive equipment. Ernest made consistent efforts to achieve all long-term and short-term goals during his stay at Cortlandt Healthcare.

Ernest was discharged to his home on March 24, 2023, after 17 days of rehabilitation. Ernest had a wonderful experience throughout his stay, making friends with both staff and other residents. Edmond was admitted to Hudson Valley Hospital after officially being diagnosed with a stage 4 pressure ulcer of the sacral region. The definition of a pressure ulcer is damage to an area of the skin caused by constant pressure for a long period of time, which can result to causing an open wound. Edmond was evaluated by Hudson Valley Hospital very thoroughly and recommended physical and occupational therapy to regain his prior level of function. Edmond was recommended to Cortlandt Healthcare for recovery due to our outstanding rehabilitation program. Edmond’s main goal was to be able to get stronger and to walk again.

Upon arrival, Edmond was greeted by the Concierge, Nursing, and Rehab Team, who ensured he was supplied with everything he needed to start his journey here at Cortlandt Healthcare, which included proper food/dietary options, making sure he received and was brought to recreational activities when wanting to attend, and making sure he was receiving medications on time. Edmond was assessed by our Rehabilitation team and received short and long-term goals to monitor his progression during her rehab process. Physical Therapy goals included pivot transfers, ambulating on level surfaces, ascend and descend stairs, and bed mobility tasks. Occupational Therapy’s short-term and long-term goals include standing balance, toileting tasks, and practicing LB dressings.

Starting with Physical Therapy, Edmond progressed to total independence with pivot transfer from maximum assist supervision to no assist. He was able to ambulate on level surfaces starting at 0 feet and ending at 50 feet with max assist. Edmond was able to ascend and descend from 0 to 5 stairs using a handrail. Edmond also progressed from maximum to independent on all bed mobility tasks and was able to perform car transfers from 0 to contact guard assist. Working hard with Occupational Therapy, Edmond went from being unable to stand to fair with minimum assist. Edmond went from minimum assist to total dependence on toileting tasks and maximum to minimum on Lb dressings due to weakness on the right side. Edmond made consistent efforts to achieve all long-term and short-term goals during his stay at Cortlandt Healthcare.

Edmond was discharged to his home on October 4, 2022, after 355 days of rehabilitation. It was a long road to recovery, but Edmond was determined and strived to get back to his normal self. He had a wonderful experience throughout his stay, making friends with both staff and other residents. Marjorie was admitted to St. Luke’s Hospital for hip surgery. The hospital post-surgery evaluated her, and the recommendation was for physical and occupational therapy to regain her prior level of function. Marjorie was referred to Cortlandt Healthcare for recovery due to our outstanding rehabilitation program. Her main goal was to be able to function independently once again and to get back home as soon as possible.

Upon arrival, Marjorie was greeted by the Concierge, Nursing Team, and Administration, who ensured she had everything she needed. The concierge assisted by ensuring Marjorie had everything she required during her stay, including proper food options, being brought to recreational activities, ensuring her medications arrived on time, and being properly taken care of during her stay. Our dietician assessed Marjorie and discussed certain food options she would enjoy during her stay. She was also assessed by our Nursing staff as well as our Rehabilitation team and received short and long-term goals to monitor her progression during her rehab process. Short and long-term goals include bed mobility tasks, safely ambulating on level surfaces, ascending and descending stairs, performing toileting tasks, and car transfers.

During Marjorie’s stay, she received both Physical and Occupational therapy. Marjorie progressed from moderate assist to minimum assist on bed mobility tasks without the use of side rails. She was able to safely ambulate on level surfaces starting at 90 feet and 350 feet maximum. Marjorie was able to ascend and descend from 0 to 4 stairs. She also performed toileting tasks from maximum assistance to minimum assistance. Working hard with Occupational Therapy, Marjorie safely performed functional activities starting at 5 minutes to 10 minutes maximum. Marjorie went from maximum to minimum on all indoor home maintenance tasks and consistently achieved all long-term and short-termshort-term goals during her stay here at Cortlandt Healthcare.

Pauline was admitted to Hudson Valley Hospital after officially being diagnosed with an Acute Embolism and Thrombosis in her left leg. Acute Embolism and Thrombosis are when a blockage (blood clot) is formed in body parts and can travel to the lung. Pauline was thoroughly evaluated by Hudson Valley Hospital and recommended physical and occupational therapy to regain her prior level of function. Due to our outstanding rehabilitation program, Pauline was recommended to Cortlandt Healthcare for recovery. Pauline’s main goal was to be able to function independently once again and to get back home as soon as possible.

Upon arrival, Pauline was greeted by the Concierge and Nursing team, who ensured she had everything she needed. The concierge helped by providing that Pauline received everything during her stay, including proper food options, that she had Recreational Activities available, and that her medications arrived on time. Pauline was assessed by our Rehabilitation team and received short and long-term goals to monitor her progression during her rehab process. Short and long-term goals include stand pivot transfers, stair assessment, bed mobility, and car transfers.

During Pauline’s stay, she received both Physical and Occupational therapy. Pauline progressed from maximum assist supervision with all stand pivot transfers. She could ascend and descend from 0 to 8 stairs and ambulated 0 feet to 50 feet under supervision. Pauline also progressed from maximum to independent on all bed mobility tasks and was able to perform car transfers from 0 to contact guard assist. Working Hard with occupational Therapy, Pauline went from maximum to minimum on all indoor home maintenance tasks. Regarding toileting tasks, Pauline went from maximum to minimum. During her stay at Cortlandt Healthcare, Pauline made consistent efforts to achieve all long-term and short-term goals.

Eileen was admitted to Westchester Medical Center after being officially diagnosed with Guillain-Barré syndrome. GBS is a rare, autoimmune disorder in which a person’s immune system damages the nerves, causing muscle weakness and sometimes paralysis. At Westchester Medical Center, Eileen underwent surgery on her back/spine. Doing thorough research, Eileen and her partner Mike decided to choose Cortlandt Healthcare for recovery due to Cortlandt’s incredible rehabilitation program. Eileen’s main goal during her stay here was to maintain independence once again and be home with her family.

Upon arrival, Eileen was assessed by our interdisciplinary team, which includes nursing, Concierge, recreation, and dietary. Eileen was immediately greeted by Dominic, Concierge, who was able to help make Eileen’s experience more comfortable and easy. Dominic was an outlet to make sure Eileen received everything she needed during her stay, including food options, making sure her medication was given at specific times, making sure recreation gave her independent activities and avid participation in group activities as well and providing weekly evaluations to make sure her needs are met. Eileen was assessed by rehab and was given short and long-term goals to monitor her progress during her rehabilitation process. These short and long-term goals include bed mobility, balance, toileting, and stair assessment.

During Eileen’s stay, she received Physical Therapy and Occupational Therapy. Eileen progressed from maximum assist to minimum assist with all bed mobility tasks and moderate to minimum functional transfer tasks. Working hard with Occupational Therapy, Eileen went from poor to fair in standing balance and total dependence to stand by assist for toileting tasks. Eileen ascended and descended from 0 to 6 stairs total and ambulated 40 feet with moderate assistance to 60 feet with minimum assistance. After three weeks, Eileen required maximum assistance for minimum aid on most activities. Throughout her journey at Cortlandt Healthcare, Eileen made consistent efforts to achieve all short-term and long-term goals during her stay. Eileen was discharged to her home on February 18 after 35 days of rehabilitation. Right before her departure, Eileen was able to host and teach a yoga art class for the residents at Cortlandt. She showcased her talents with the Cortlandt community, and it was a great experience for everyone.

Details of Experience:
Nicholas A. was at home when he had some lower back pain. Not thinking anything of it, the pain got worse each day. When realizing something was wrong, he went to the doctor, who ordered an MRI and CT scan. He was then admitted to the Mount Sinai Hospital in New York City, as he was found to have an acute compression fracture and spinal stenosis. Mr. A. underwent a Kyphoplasty to treat the fracture in his vertebra and a Decompressive Laminectomy to treat the lumbar spinal stenosis. This also relieves pressure on the spinal roots.

After his surgeries, Mr. A. found out he needed to go to rehab to regain his strength and balance. He knew he wanted to come to Cortlandt Healthcare because he has been with us in the past. He was admitted to Cortlandt on September 16th, 2020 for Occupational and Physical Therapy and was greeted by our Nursing and Concierge Team. He was evaluated by Therapy the next day to begin his rehabilitation journey.

While being evaluated by our therapy team, they developed short term and long term goals for Nicholas. Some of these include, safely performing bed mobility tasks, performing functional transfers, safely going up and down four stairs using handrails, completing hygiene and grooming tasks, increasing sitting and standing balance to reduce the risk of falls and performing functional mobility during activities of daily living. His personal goal was, “I want to get stronger so I can go home.”

Nicholas was able to achieve his goals by using different treatment approaches during therapy, including: therapeutic exercises and activities, neuromuscular reeducation, self care management, wheelchair management, gait training therapy and manual therapy techniques. He also mentioned that he loved working with his therapists, Sean and Mercedes. The patient stated, “They helped him so much and guided him through everything.”

Mr. A. is now independent in all the areas he needed assistance with and he is readying to return home to his family and excited to be able to do the things he was able to do before. He thanks Cortlandt Healthcare for helping him along his journey.

Details of Experience:
Sylvia Forsmith was just getting out of bed when she fainted and fell to the floor. She was on the floor for an hour and a half before her husband returned from the store and found her. She couldn’t call him herself because the phone was out of reach and she was helpless on the floor. When he found her, he called 911 immediately. An ambulance soon showed up and took Ms. Forsmith to Putnam Hospital Center.

While at the hospital, she was checked for injuries and got several X-Rays done to make sure she didn’t have any broken bones. What they did find was she had Congestive Heart Failure. When Sylvia found out she needed to go to rehab, she knew she didn’t want to go back to the place she was at before. She needed a place where people are kind and willing to help with whatever she needed. This was when she found out about Cortlandt Healthcare. Mrs. Forsmith thought it was so wonderful that our admissions team put in a lot of time reassuring her, saying Cortlandt was the right fit for her. From this moment on, Sylvia knew that we would be the ones to take good care of her to help her get the help she needed.

When Sylvia got to Cortlandt, she was greeted by our nursing staff and concierge to make sure she felt comfortable right away. During her first night with us, she started to feel anxious, but our night supervisor, Mon, helped her feel relaxed. Going forward, she didn’t have any problems because she knew she was in good hands. Sylvia shared, “Cortlandt Healthcare is so much better than the other rehab I was at before. Not a single person is negative here and all the CNAs are just friends that want to help you! They’re so reassuring and great!” She also thought Millie and Nicole were so compassionate and sweet and she loved to start her day out seeing them.

After Mrs. Forsmith was evaluated with our rehab team, she was assigned therapists for Physical and Occupational Therapy. This was when they created her short and long-term goals. These include facilitating independence with self-care tasks; increasing sitting balance during activities of daily living (ADLs); safely performing bed mobility tasks with the use of side rails to get in and out of bed; performing functional transfers and decreasing risk for falls; and regaining her prior level of functional abilities. Nancy and Cori were her therapists and Sylvia loved them! She thought they were so encouraging even when they weren’t in therapy. They wanted to keep her motivated to keep working towards her goals. At first, they worked on a lot of little things to help her hands. They did lots of arm exercises to get some of her upper body strength back. Once they had some upper extremity improvements, they moved to her lower extremity so she can gain strength back in her legs and be able to walk more independently. Sylvia worked really hard during her rehab sessions and noticed it got harder each time but she kept pushing and she can now walk up and down the hallway! She is now working on her transfers so she can become independent and go home to her husband.

Sylvia Forsmith is a retired New York City public school teacher. Her favorite grade to teach was 3rd grade. She retired in 1996 to her home in Carmel, New York where she and her husband of 55 years have lived together. After retirement, she enjoyed gardening and taking care of her many cats and dogs. During her free time here at Cortlandt, she enjoyed painting and doing other crafts with Luz, and she enjoyed her Urban Zen sessions.

Details of Experience:
After moving in with his family to help watch his grandkids, Joe A. realized that after a while, his grandkids were watching over him rather than Joe watching over them. Henry Calam was a tennis player and wrestler at Princeton University. After joining the NROTC, he joined the Navy in 1958. He met his wife, got married and settled down in Ossining, New York. One Tuesday morning, Henry tried to get up out of bed and ended up falling onto a hard wood floor and he could not move. He fell between the wall and the bed and needed immediate assistance. His wife found him, called 911 and Henry was sent to the Emergency Room at Hudson Valley Hospital Center. After getting several X-rays, Mr. Calam fractured his shoulder in three places. He did not need surgery but needed hands-on care for it to heal properly. Henry’s daughter knew about Cortlandt Healthcare and together they chose Cortlandt Healthcare for his Rehabilitation needs.

When Mr. Calam arrived at Cortlandt Healthcare, he was greeted by our nursing staff, rehabilitation team and concierge director. Henry liked the gym, and he loved his therapists. Some of his short-term and long-term goals included hygiene and grooming tasks, sitting on the edge of his bed balanced, self-feeding, increase standing balance to reduce risk for future falls and other activities for daily living.

During therapy, he worked on his upper body to work his shoulder and make it stronger. He used a lot of the bands and the weights then started with some of the machines. After they worked on his upper body, they moved to his lower body to work on his balance. Thanks to the individualized treatment plan and Henry’s hard work, he reached his goals of mending his fractures and growing back the muscles in his upper body. When asked about his therapists he said, “Ricky, Mike, Lisa, Mercedes and Sean all did such a great job and I loved working them each and every one of them.” He currently uses a walker to ambulate.

On August 28, 2020, Henry discharged home. Being a former athlete, Henry had a passion for coaching and taught tennis in Ossining, New York. Even in his retirement he enjoyed golf and it was hard for him to sit still after his injury. Henry enjoyed the staff and will miss everyone that worked with him. “Everyone was very considerate, pleasant to work with and very aware of everyone’s personal needs.”
